Resident Evil Requiem has five locked safes, all opened by Grace, and their combinations are fixed rather than random: on Casual and both Standard modes they are the same on every playthrough, so you can dial them in without ever finding the code files. Insanity swaps in a different set of codes for all five safes, which we list separately below. Every code here was checked against at least two independent guides, and where those guides disagree we say so.
We cross-checked each combination in PowerPyx's safe guide, Game8's safe code pages and IGN's safe codes guide, plus IGN's Insanity notes and PowerPyx's trophy guide, in November 2026.
All Five Safe Codes at a Glance
Two safes sit in the Rhodes Hill care center, one in its basement, and two in the ARK facility near the end of the game. Each combination is three numbers with a direction for each turn. PowerPyx, Game8 and IGN agree on every Casual and Standard code except one dial direction noted under the table, and PowerPyx and IGN agree on all five Insanity codes.

| Safe | Area | Casual and Standard | Insanity | Contents |
|---|---|---|---|---|
| Bar and Lounge | Care Center, 2nd floor | Left 10, Right 80, Left 30 | Right 20, Left 50, Right 30 | 3 Antique Coins |
| Examination Room | Care Center, East Wing | Right 30, Left 10, Right 50 | Left 50, Right 30, Left 90 | 3 Antique Coins |
| Basement | Care Center Basement | Right 60, Left 40, Right 80 | Left 70, Right 50, Left 20 | Handgun ammo, Raccoon Roundup Map |
| Sterilization Chamber | ARK | Right 10, Left 90, Right 20 | Left 80, Right 50, Left 0 | Med Injector, Molotov Cocktail, handgun ammo |
| Monitor Control Room | ARK | Left 50, Right 60, Left 80 | Right 20, Left 0, Right 70 | Med Injector, Empty Bottle, Bottle of Acid |
The one disagreement: IGN lists the standard Monitor Control Room code as 50 right, 60 left, 80 right, with every direction reversed compared to PowerPyx and Game8. Two of three sources give left 50, right 60, left 80, so start with that and try the reversed version if the safe does not open.
Are the Codes Fixed, or Do They Change?
They are fixed per difficulty. PowerPyx states that the safe codes are always the same and that you can open each safe even if you have not read the file that contains its combination. That makes the standard codes reliable on any Casual, Standard (Modern) or Standard (Classic) playthrough, first run or fifth.
Insanity is the exception. PowerPyx, Game8 and IGN all say every safe has a new combination on Insanity, and IGN and Game8 add that the files giving those combinations stay in the same places. The Insanity column above is where PowerPyx and IGN match code for code. Game8's Insanity tables are not reliable: its dedicated Insanity page lists the standard codes for two safes and swaps others around, and it even contradicts Game8's own main safe page. If an Insanity code fails, read the in-game file for that safe; it will always show the correct combination for your difficulty.
Even when you already know the numbers, pick the code files up. They are part of the 75 files that count toward the Case Closed trophy, according to PowerPyx, and skipping them now means another trip later.
Other locks behave differently. IGN notes that the non-safe combinations, such as the S.T.A.R.S. office briefcase and the symbol puzzle boxes, use the same solution on Casual, Normal and Insanity. Our puzzle solutions guide covers those.
How to Enter a Combination
Each safe uses a dial. According to PowerPyx, you turn the dial left or right onto each number in sequence, then press the confirm button to open it. If a number or a direction is wrong, the dial spins back to its starting position and you can simply try again, so there is no penalty for a mistake beyond the time it takes.

- Follow the direction exactly. A correct number turned the wrong way still fails, which is why the Monitor Control Room disagreement matters.
- Grace only. IGN notes that Leon cannot interact with safes at all, even though he passes through some of the same rooms.
- Clear the room first where you can. Several safes sit in rooms with enemies, and the dial interaction leaves you exposed.
- One safe is a freebie. IGN points out a sixth safe in the Rhodes Hill waiting room that is already open when you arrive, and it does not count toward the five.
The Rhodes Hill Safes
Bar and Lounge is the first safe most players see. It sits behind the bar counter in the Bar and Lounge on the second floor of the care center, in a room with three zombies including a singing one, according to IGN and PowerPyx. You pass through it on the story path, so it is easy to grab early if you can deal with the room. Its code file is on a desk in the nearby Office, per IGN, but you do not need it. If Grace is short on ammo, IGN suggests coming back after Leon has cleared the room later in the care center chapters.
The Examination Room safe is in the operating theater area of the East Wing's Examination Room, inside a cabinet in the corner, and IGN says its code file is in the room where the Chunk first appears. Both of these safes hold three Antique Coins each.
The Basement safe is the odd one. It is in an unlabeled room near the forklift in the care center basement, reached with a Joint Plug or through a crawlspace, per IGN. The code file cannot be found by Grace at all: Leon finds it in a warped cabinet in the attic, which only his hatchet can open, and Grace can then read it from the inventory screen. Our Leon guide explains how his chapters overlap with Grace's in the care center. Inside you get handgun ammo and the Raccoon Roundup Map, which marks most of the Mr. Raccoon memoriams in Rhodes Hill on your map.
The ARK Safes
The last two safes come late, during Grace's return in the ARK facility. The Sterilization Chamber safe sits under the window on the room's counter, surrounded by broken glass, and the room is patrolled by Lickers, so IGN and PowerPyx both recommend sneaking. IGN notes its code file is only found after you leave, in an open storage container south of Bioweapon Repository 05, which is why blind players often have to backtrack. Knowing the code in advance saves that trip.
The Monitor Control Room safe is on a bench against the back wall of the Monitor Control Room, outside the Archives, according to IGN, and PowerPyx describes reaching it after using the Magnetic Key on the red door by the upstairs typewriter. Its code file is in a container in Bioweapon Repository 05 that IGN describes as sitting between two patrolling Lickers with a zombie inside, another good reason to use the code from the table instead. Game8 notes this is the last safe in the game and can be opened toward the end of the story.
Opening the Safes on Insanity
Insanity is where safes trip people up, because muscle memory from a first run gives you the wrong numbers. IGN's Insanity notes and PowerPyx's roadmap flag a few things that make the safe route smoother on the hardest difficulty:
- Use the Insanity column above, or read each safe's file again; the files are in the same places as before.
- Expect harder rooms. IGN says Grace dies within two hits from most enemies on Insanity and that there are more enemies overall, so the crowded Bar and Lounge and the Licker rooms in ARK are more dangerous than you remember.
- Bring the bonus weapons. PowerPyx's platinum route buys Infinite Ammo and the infinite rocket launcher before an Insanity run, and notes that Infinite Ammo lets Grace fire the Requiem revolver freely.
- Do not count on the Raccoon Roundup Map. IGN says the Basement safe on Insanity holds handgun ammo, and the map only appears there if you never picked it up on an earlier run.
- Back up a clean save before you start. A copy of your progress outside the game protects a long Insanity run; our save file location guide shows where it lives on each platform.
Our difficulty guide covers the rest of what changes on Insanity.
Why the Safes Are Worth Opening
Six of Grace's Antique Coins come from the two care center safes, and those coins matter. IGN explains that Antique Coins are Grace-only and are spent in the parlor on upgrades such as a pouch that adds two inventory slots, a health upgrade, a gun stabilizer and a larger blood capacity for crafting. Leon cannot pick them up, so missing them in Rhodes Hill means missing upgrades for the rest of Grace's chapters.
Opening all five also unlocks the No Safe is Safe trophy, which IGN notes also counts as a challenge in the Bonuses menu. On Insanity, IGN adds that some Antique Coins move to new locations and some bosses drop them instead of their usual loot, so coin routes from your first run will not fully carry over. For the full list of what challenges unlock, see our unlockables guide, and for how the safes fit into a full clear, our Requiem length guide.
Skip the Searching on Your Next Run?
If you have already opened every safe once and would rather start your next playthrough fully equipped, the Resident Evil Requiem modded save starts you with every weapon unlocked and upgraded to maximum, infinite ammo, all character upgrades maxed in the shop, 500,000 credits and all challenges completed.
It is a PS5 and PC save only; the product page rules out Xbox and Nintendo Switch 2, and you still need your own copy of the game. The platinum trophy service is sold separately on the Resident Evil Requiem store page.
Frequently Asked Questions
Are Resident Evil Requiem safe codes random?
No. They are fixed on Casual and Standard and the same on every playthrough. Insanity uses a different fixed set for all five safes.
How many safes are in Resident Evil Requiem?
Five that count for the No Safe is Safe trophy: Bar and Lounge, Examination Room, Basement, Sterilization Chamber and Monitor Control Room. A sixth safe in the waiting room is already open.
What is the Bar and Lounge safe code?
Left 10, Right 80, Left 30 on Casual and Standard. On Insanity it is Right 20, Left 50, Right 30.
Can I open a safe without finding the file?
Yes. The combination works whether or not you have read the file, so you can enter it as soon as you reach the safe.
Can Leon open safes in Resident Evil Requiem?
No. Only Grace can interact with safes, although Leon is the one who finds the Basement safe's code file in the attic.
Why is my safe code not working?
Check the direction of each turn and your difficulty. Insanity codes differ, and the standard Monitor Control Room code is listed with opposite directions by IGN, so try the reversed sequence if the usual one fails.
