Resident Evil Requiem Puzzle Solutions: Every Answer

Resident Evil Requiem YouTube thumbnail: Resident Evil Requiem - All Puzzle Solutions

Below are the solutions to every main puzzle in Resident Evil Requiem, from the Chairman's Office symbol box to the post-game Final Puzzle. Puzzle answers are fixed: PowerPyx says they are the same on every difficulty except the safe codes, and IGN confirms the non-safe locks use the same solutions on Casual, Normal and Insanity. Each answer below appears in at least two of the guides we used, and the one place they describe a step differently is flagged.

We confirmed these solutions against PowerPyx's puzzle guide, Game8's puzzle pages and IGN's Requiem wiki in November 2026.

Quick Answers for Every Puzzle

If you only need the answer, the table has it. Most of Requiem's puzzles sit in Grace's half of the game at the Rhodes Hill care center, with a couple of optional ones in Leon's trip through the R.P.D. The sections after the table explain where each puzzle is, how the clue works and what you get, in case you want to understand the logic rather than just enter it.

Safes are covered separately, because they are the one lock that changes on Insanity. Our safe codes guide lists all five combinations for every difficulty.

The Care Center Symbol Boxes

Two of the three Quartz Grace needs to leave the care center come from symbol boxes, where you press Sun, Moon and Star buttons in a set order. The Chairman's Office box is on the second floor of the West Wing. PowerPyx explains the clue: pick up the Pencil in the back room of the office and shade over the sheet of paper on the desk to reveal the order. The answer is Moon, Sun, Star, Moon, and Game8 lists the Moon Quartz as the reward.

The Lead Researcher's Office box is on the second floor of the East Wing, behind a padlock. PowerPyx says to fetch the Corrosive from the Conference Room at the end of the hallway outside the office and use it to melt the lock, then look at the photos around the room, whose backgrounds show the symbols. The answer is Star, Sun, Moon, Sun, and the box holds the Sun Quartz plus the Leatherbound Notebook. That notebook matters: IGN notes it contains the instructions for the Organ Transport Box, so do not leave it behind. The same office returns at the very end of this guide, because the Final Puzzle is entered on this box.

The Organ Transport Box and the Organless Corpse

These two puzzles chain together to earn the ID Wristband (Level 2). IGN's guide starts in the Examination Room in the East Wing, where a corpse on the table is missing its heart and lungs, and a screen nearby shows which organs to find. You need the ID Wristband (Level 1) from the Lead Researcher's Office before any of this opens up.

The lungs are in the Organ Transport Box, sitting in the back of a truck at the far end of the Garage in the West Wing, through the Level 1 security door near the kitchen. Inspect the box, find the removable back panel, and follow the diagram from the notebook. PowerPyx and IGN give the same order:

  1. Turn the dial to Hand.
  2. Raise the slide switch on the right to the middle position.
  3. Flip yellow switch 5 down.
  4. Flip yellow switch 2 down.
  5. Turn the dial back to Auto.

Game8 describes the same settings, finishing with the dial turned to Hand and then back to Auto, and says the steps do not need a specific order. IGN adds that a reset switch puts everything back if you get it wrong. Opening the box triggers a truck event in the garage; IGN says to shoot the driver to stop it and squeeze under the wheels.

For the heart, IGN says to take the Wrench from the Custodian's Office, use it on the door without a handwheel at the far end of the West Wing, and collect the Artificial Heart in Cold Storage. Back in the Examination Room, insert both organs. The corpse then comes back to life and attacks, so lead it into the hallway if other enemies are still nearby, defeat it, and take the ID Wristband (Level 2) it carries.

The Blood Analyzer Puzzles

Grace's crafting depends on analyzing blood specimens at a laser microscope, and each analysis is a small logic puzzle. PowerPyx's trophy guide describes the goal as turning every atom red, and solving the first one unlocks new crafting recipes along with the Science! trophy. Two of the solutions are described identically by PowerPyx and Game8:

  • Blood Specimen (Converged): click the top middle square, then the second square from the bottom.
  • Blood Specimen (Reversible), in the Blood Lab side room behind the wristband door: click the bottom-left corner square, the middle square, the top-right square, then the second square from the top.
  • Blood Specimen (Denatured), on the Blood Lab counter: a single click, but the guides word it differently. PowerPyx says the left middle square and Game8 says the second square from the left, so try those squares if your first click does not finish it.

Insanity adds a fourth sample. PowerPyx lists a Blood Specimen (Polymerized) where the Converged sample sat on lower difficulties, solved by clicking middle left, middle right, then middle bottom. We could not confirm that solution in a second guide, so treat it as unverified until you see it work.

The VIP Suite and the Hourglass

After Grace escapes the care center, the helicopter pilot in the courtyard sends her to find a Helicopter Key, and the route runs through the VIP Suite, according to IGN. The room is full of objects you can inspect, but only one matters. Inspect the red wine bottle on the left, rotate its misaligned label until the logo lines up, and the bottle opens to reveal the Double Helix Model. Place it in the podium in the middle of the room, and the whole suite descends to the next area. PowerPyx gives the same solution.

PowerPyx notes the hourglass on the desk is not part of this puzzle, even though you can pick it up. It is not useless, though. IGN found that flipping it upside down while inspecting it reveals a message under the sand, "U = 380,000", which is one of the clues for the Final Puzzle described at the end of this guide.

Optional Puzzles: Briefcase, Barry's Hunt and the Roulette Wheel

Three side puzzles reward charms or coins. The roulette wheel is in the Parlor in the care center's West Wing, which Game8 says you open with the Red Jewel from the Unicorn Trinket Box in the Chairman's Office. According to IGN and Game8, the wheel only ever lands on 7, 15 or 29: shoot one or more of those numbers on the table, spin, and if the ball lands on a number with a bullet hole you get an Antique Coin. It pays out once only.

The other two are Leon's, in the R.P.D. The briefcase in the S.T.A.R.S. Office on the second floor opens with RRR and holds the Power Shades Charm, per Game8 and IGN. Barry's scavenger hunt starts with two photo clues in the same office, one in the locker right of the door and one in a jacket between the desks. They point to the Operations Room on the first floor: climb the wooden box near the whiteboard to find a Locker Key on the TV stand, then open the locker in the West Office for the S.T.A.R.S. Emblem Charm, which IGN says boosts firepower for automatic guns at the cost of accuracy. Our Leon guide covers what else happens on his return to the station.

The Final Puzzle (Spoilers)

Spoiler warning: this section mentions late-game areas and characters. The Final Puzzle is a hidden challenge that only becomes readable after your first clear. Its whole description is "Let the sweet pair hear the voice", and both IGN and PowerPyx say it pays 20,000 CP. It needs two playthroughs, so check our Requiem length guide before you commit.

  1. Get Marie's Doll on a run. IGN's steps: in the care center basement processing room, wait about 15 minutes for 115 infected to be processed, then drain it; during the grinder sequence that follows, kill none of the zombies yourself; after draining the blood vat the message must say a ton of blood flowed out. Then flush a toilet in the West Wing bathroom eight times. The doll then appears on the stairs of the Blister Head room during the facility's self-destruct. Keep it in your item box and finish the game.
  2. Start a new playthrough. The doll carries over in the item box.
  3. After the basement, analyze the Severed Hand at a laser microscope. It shows "Let's play" and the sequence GGC AAG AUA ACG UGU CAU.
  4. Decode it. IGN maps Moon to U (the hourglass clue, about 380,000 km), Star to C (4.2 light years, from Grace's Blood Analysis Report) and Sun to G (150,000,000 km, from a sticker on the Blood Lab microscope). A Toy Uncle bobblehead in Raccoon City says to ignore A, leaving GGC GUC GUG UCU.
  5. Carry Emily, have Marie's Doll in your inventory, and enter Sun, Sun, Star, then Sun, Moon, Star, then Sun, Moon, Sun, then Moon, Star, Moon on the Lead Researcher's Office symbol box. Game8 lists the same code.
