How to Become a Ghoul in Fallout 76: Full Guide

To become a ghoul in Fallout 76, take a character to level 50, tune into the Distress Signal radio station to start the quest Leap of Faith, and see it through to its follow-up, A Fresh Pair of Genes, where you choose to walk into an irradiated silo and emerge as a ghoul. The option arrived with the Ghoul Within update on 18 March 2025 and is still the only route as of November 2026. Below is the full walkthrough, how Glow and the Feral meter work, what you give up, and how to switch back if ghoul life is not for you.

We checked this against Bethesda's Ghoul Within release notes, the Fallout Wiki's Leap of Faith, A Fresh Pair of Genes and Ghoulification pages, and Bethesda's Burning Springs and Infestations notes, in November 2026.

Requirements and What You Are Signing Up For

The Fallout Wiki's Ghoulification page, citing Bethesda's own guide to playing a ghoul, says characters that have reached level 50 can choose to become a ghoul during A Fresh Pair of Genes. If your character is close, finish the last few levels first so the offer is waiting when you reach Parthenia; the Level 50 Character Boost that Bethesda introduced alongside Ghoul Within is another way to get a fresh character there.

The trade is significant. Bethesda's release notes describe ghouls as immune to disease and unable to be mutated by nearby radiation; instead, radiation fuels a new resource called Glow. Hunger and thirst disappear and are replaced by a Feral meter. You also gain access to 30 ghoul-exclusive perk cards, 28 regular and 2 legendary, which you can use alongside the normal human cards.

  • Gained: disease immunity, Glow, the Feral meter, 30 ghoul-only perks and extra cosmetic options such as complexions, tattoos and scars.
  • Lost: hunger and thirst perks, Chem Resistant and several radiation perks, plus 10 points of Charisma.
  • Complicated: some factions, including the Brotherhood of Steel, turn hostile.

Leap of Faith Walkthrough

Spoiler warning: this section describes the quest's story beats and the choices you make about two characters' fates.

  1. Tune in. Open your Pip-Boy radio and listen to the Distress Signal station, then travel to the Whitespring Refuge.
  2. Meet Leamon Price. Inside the Refuge's medical area, Leamon is suffering from radiation sickness. The Responder medic says his only hope is a radiation specialist at Hillside Cavern to the northeast.
  3. Reach Hillside Cavern. In the tunnels, a ghoul named Kevan Asherton stops you and leads you and Leamon to Parthenia Blankenship, who can save Leamon by turning him into a ghoul, but needs Irradiated Ore.
  4. Fetch the ore. Go with Asher to the Emmett Mountain Disposal Site. A trapdoor in a shack on the southern side leads to the objective; unlock the gate and hold off waves of feral ghouls while Asher collects the ore.
  5. Settle the dispute. Madeline Keene, a scavenger, argues with Asher. You can use Strength 8 or higher to force Asher to back off, Charisma 8 or higher to talk him into letting her leave, or encourage him to kill her.
  6. Help Leamon decide. Back at Hillside Cavern, Parthenia favors ghoulification and Asher opposes it. Encouraging Leamon to change makes him a member of the Revenants; encouraging him to stay human means he accepts his fate.

When Leamon's story ends, Parthenia asks to speak with you, which starts A Fresh Pair of Genes. Leap of Faith itself rewards the Radiation Quack outfit.

A Fresh Pair of Genes: The Transformation

This short quest is where you make the actual decision. Talk to Parthenia about becoming a ghoul and she explains the process; one dialogue option opens a popup listing the benefits and downsides, which is worth reading before you commit.

If you go ahead, pick up the inhaler on the table and enter the hallway behind Parthenia. It leads to an irradiated silo, where the radiation knocks you out and the character customization menu opens so you can design your ghoul. When you wake up, you are free to explore Radiant Hills and meet the ghouls who live there. The quest rewards two doses of The Fix, a chem that matters a great deal to ghouls.

Early bugs have been addressed. The wiki notes that characters with very low health were once killed by the radiation during the transformation, which a later patch fixed, and Bethesda's Ghoul Within hotfix stopped players from getting stuck as human after completing the quest. If you decline the transformation, A Fresh Pair of Genes simply never marks itself complete, so you can come back later.

Glow and the Feral Meter

Glow is the ghoul's signature mechanic. When you are at full health, excess radiation turns into Glow, which acts like a second health bar shown as a green-striped pattern on your HP bar, and you can store as much Glow as your maximum HP. Irradiated food and drink, radioactive areas and self-irradiating weapons such as the Gamma Gun all add Glow, while RadAway, decontamination showers and the Rad Sponge perk remove it.

Radiation zones become a resource rather than a hazard, which makes nuke zones especially interesting for ghouls; our nuke launch walkthrough explains how to create one. The Infestations update also gave player ghouls -50 radiation resistance by default, so Glow builds up faster, and buffed Rad Specialist so it adds 10%, 20% or 30% more environmental Glow per rank.

The Feral meter replaces hunger and thirst, drains over time, and only refills when you take chems, with The Fix restoring the most. Its bands, from Bethesda's notes, decide your bonuses:

  • Above 80%: +3 Strength, +3 Endurance and +30 max HP.
  • Above 60%: +15 max HP.
  • Above 40%: -1 Endurance and -5 max HP.
  • Above 20%: -3 Endurance, -15 max HP and -10 max AP.
  • At 0%, fully feral: +150% melee damage, but -5 Endurance, -99 Charisma, -30 max HP, -20 max AP and -300% hip-fire and V.A.T.S. accuracy.

Ghoul perks shape the meter too. Chem Diet makes chems more effective at holding off the feral state, and Moral Support slows it by half while you are on a team. Our builds guide for Season 26 covers how ghoul perks fit into a melee setup that deliberately uses the feral bonus.

What You Lose and How to Work Around It

Ghouls take a -10 penalty to Charisma, which hurts Charisma perks and dialogue checks with every NPC. The Fallout Wiki notes that chems can offset the base Charisma loss, though not the much larger feral penalty. You also lose access to a list of human perks, including Ghoulish, Radicool, Rad Resistant, Lead Belly, Iron Stomach, Slow Metabolizer, Thirst Quencher and What Rads?.

Faction hostility is the bigger practical problem. The Brotherhood of Steel is hostile by default, and some interiors belonging to the Settlers and Crater Raiders become inaccessible, which can block quest lines. The fix is a disguise from Jaye Vo, who you meet in Radiant Hills, and Jaye can also remove it. Feral ghouls stay hostile no matter what, even when you go fully feral yourself. Those settlement restrictions matter for shopping too, since our Minerva guide shows that three of her four stops are faction settlements.

Some fixes have landed since launch. Burning Springs made One of Us start correctly for ghoul characters after Bunker Buster, and made ghoul-related perk card restrictions update without leaving the server. The same update added new legendary mods from Head Hunts that interact with the feral meter, such as Lucid, which raises damage up to 40% based on how full your meter is. Power armor users should also look at the ghoul perk Rad Specialist alongside our power armor comparison, since it doubles its Glow bonus with a matching set.

Going Back to Human, and Back Again

You are not locked in forever. Bethesda's Ghoul Within notes say you can become human again through the character screen if ghoul life is not for you, but that it is a one-way ticket in the sense that you cannot replay the questline to become a ghoul again.

If a character that has already completed the questline wants to return to ghoul form, Bethesda sells a Ghoul Retransformation on the character screen for 1,000 Atoms. The wiki summarizes it simply: the first switch back is free, and later switches cost 1,000 Atoms each.

That makes testing cheap. If you are curious, transform, play a week with a ghoul build, and switch back for free if you miss your human perks. Just make the second decision carefully, because flipping again costs Atoms.

Chasing the Platinum?

Ghoul life changes how faction quests play out, and several trophies are tied to those quests, such as Steel Brethren for completing The Catalyst. If the trophy list is your real goal, the store's Fallout 76 Platinum Trophy Service page offers full Platinum completion unlocked directly on your account, for PS4 and PS5, with a choice of instant or spaced-out legit timestamps. You enter your account details at checkout, and the page notes that DLC-only trophies need the matching DLC.

The Fallout 76 hub lists the store's caps and boosting services as well.

Frequently Asked Questions

What level do you need to be to become a ghoul?

Level 50. The transformation in A Fresh Pair of Genes is offered to characters that have reached that level.

How do you start the ghoul quest?

Listen to the Distress Signal radio station, then go to the Whitespring Refuge to meet Leamon Price. That starts Leap of Faith.

Can you turn back into a human?

Yes, from the character screen. The first switch is free; becoming a ghoul again afterward costs 1,000 Atoms.

Do ghouls need to eat and drink?

No. Hunger and thirst are replaced by the Feral meter, which you refill with chems, The Fix being the most effective.

Can ghouls get mutations?

Not from radiation. Ghouls cannot be mutated by nearby radiation, so mutations only come from Serums.

Why is the Brotherhood hostile to my ghoul?

Ghoul characters are treated as hostile by the Brotherhood of Steel by default. A disguise from Jaye Vo in Radiant Hills lets you access the blocked content.
